One Stop Hypertension Clinics

This is done jointly with GP, practice nurse, and Health Care Nurse. High blood pressure patients are seen jointly, receiving all tests (blood and urine) and optimisation of tablets will be done without having separates appointments with a GP. This clinic provides a coordinated care for high blood pressure patients with a singular appointment.
